A. Proof of Health:

1. Medical certificate issued by a registered medical professional or healthcare institution

2. Hospitalization record or discharge summaries

3. Prescription documents or pharmacy receipts.

4. Insurance claims related to health expenses.

5. Diagnostic test reports bills (e.g. X-rays, blood tests)
   

B. Proof of Educational Purpose:

1. Admission letter from an educational institution

2. Course details and fee structure provided by the educational institution

3. Student ID card or enrollment confirmation

4. Academic transcripts or report cards

5. Loan approval documents from educational loan providers
   

C. Proof of Bereavement:

1. Death certificate of the deceased person

2. Funeral or cremation expense receipts

3. Obituary or death announcement in local newspapers

4. Letter from the funeral home or crematorium confirming arrangements

5. Affidavit from a family member or legal representative
   

D. Proof of Calamity:

1. Disaster declaration from relevant authorities
